Arrive at our office wearing your own cloth or surgical mask
This is for your protection and ours.
Come on your own
We would love to meet your friends and family but right now we just want to see you alone unless you require assistance per ADA guidelines. Children are currently not permitted in the office.
Complete your forms online
This reduces paperwork and your time in the office. Click the button at the bottom of this page to complete intake paperwork and consent forms.
Reschedule if you’re not feeling well
If you have had recent illness or have been exposed to someone who is sick we ask you to reschedule your non-urgent appointment.
Please arrive on time
We have eliminated the waiting room so if you’re early please press the outside buzzer to let us know you are here and we will call you at the time of your appointment to allow you to enter the office when it’s clear.
